\nAward: Certificate or<\/em> Associate<\/strong><\/p>\n

Prepare to complete criminal background check forms<\/b><\/h3>\n

All students enrolled in courses with field work components (lab hours in child care centers) must complete a criminal history statement and a notarized affidavit on the first day of class or at the orientation for distance learning courses.<\/strong><\/span><\/p>\n

These forms are legal documents that must be filled out completely and correctly. They are required even if you are doing all required field work hours at your workplace. <\/b>The information will be submitted to the Texas Department of Family and Protective Services (TDFPS) for verification as required by law.<\/p>\n

You must bring a government-issued photo ID with you to the first day of class.<\/b> Acceptable photo ID\u2019s include:<\/p>\n
